Has an employee reported sick? It is important to gain early insight into any factors that may affect recovery or return to work. That’s why, from day 5 of the sickness absence, we send the employee a short digital early signal questionnaire.
The aim is to identify at an early stage whether there are factors that could delay recovery or return to work — such as a workplace conflict, mental strain, or other barriers to employability. The questions are general in nature and focused on return to work. Any medical matters are assessed exclusively by the company doctor.
Why we use this questionnaire
- Early identification of the risk of long-term absence
- Faster involvement of the company doctor where necessary
- Prevention of escalation or unnecessary delays
- Support for proper compliance with the ‘Wet verbetering poortwachter’
Privacy and feedback
Individual responses are not shared with the employer. The questionnaire is stored in the medical file and is only accessible to the company doctor.
If an employee answers “yes” to one of the signals, we will get in touch to assess whether early involvement of the company doctor is required.
Practical information: how the employee receives the questionnaire
- The questionnaire is sent by email from day 5 onwards
- Available in both Dutch and English
- Takes only a few minutes to complete
Digital only
The questionnaire is, in principle, only available digitally. Sending it by post would cause unnecessary delays and does not align with the goal of early identification.
Upon request, we can provide a PDF version of the questionnaire. Due to the privacy-sensitive nature of the information and the need for speed, this may only be returned as a scanned document or photo via medischsecretariaat@invita.nl.
